It is important to save any information you discover in writing, including the vaccine name (e.g., "MMR"), date given, and provider or clinic name. Utilize the immunization record as a lifelong record of an individual’s immunization record.

- Remember to bring the immunization record card to all medical appointments.
- Be sure all immunization providers give you a written record of the vaccine(s) you received.
- Keep your immunization record in a safe place. You may need it throughout your life.

HSSEQ QUIZ
This month, Jamal kicked off the HSSEQ Monthly Quiz. He went to each department and ensured that their Toolbox book was up to date in order for the department to be eligible to participate. Here, he tested them on the Toolbox topics that were sent out for the month of January. The entire office was buzzing with excitement and everyone ensured that they were on top of their HSSEQ game in order to win some cool prizes.
What an innovative way to get everyone involved in and aware of HSSEQ within our company! Great job Jamal and congratulations to all our winners!
